Senior Gifts

Posted by EAST LAKE HIGH SCHOOL SWIM & DIVE TEAM at Aug 15, 2016 5:27PM PDT

Team Tradition—
Every year, the underclassmen purchase gifts for the seniors to be given out on Senior Night. Underclassmen please bring in $10 to practice or to the pancake breakfast on Friday and give to Jacob, Ethan, or Chase. It’s always fun to see the gifts on Senior Night.

We have a lot of people who are not on Team Pages yet, please pass information on.

Thank you!

Coach

It was great to see some of you at practice; however, it is required by the high school that we correct all paperwork before swimming. As of right now we only have one swimmer, Sarah Savage who has completed all required paperwork. Kudos to you Sarah.

Required Paperwork 2016:
The Pinellas County School Board (PCSB) and FHSAA set the rules for participation in High School Sports and Activities. All athletes must have turned in proper paperwork before they can participate in any activities—even practice.

Athletic Forms
All required paperwork will be available on this web site in the “Documents” section. Click on “documents” on the menu bar, then on “Swim Documents 2016”. This package consists of several sections:
• Complete the pages of info on the student. This section contains a signature block that must be notarized.
• There are a couple of pages that must be completed and signed by the student’s health care provider to clear the student for athletic activity.

Pinellas County School Board Packet: http://www.pcsb.org/cms/lib8/FL01903687/Centricity/domain/176/pcs%20forms/4-1891-A.pdf

Insurance
All high school athletes in Pinellas County must buy the PCSB-approved athletics insurance policy. It is not expensive (about $6). This covers all High School sports (although there are extra charges for football). It can be purchased online and then the receipt should be attached to the Athlete Info packet.
If you want more information on the coverage and the PCSB requirement, see these links:

Concussion

Concussion Video (http://www.nfhslearn.com/) (students must create an account, log in, and enroll in the Concussion in Sports for students Course. Print out the form upon completion and bring to coach)

Please copy and paste the links above or go to Team pages “Required Paperwork” and click on the links.
